Pay Rate: $74.00

Position Summary : Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning. This role will report to the practice manager.

Duties :

• Patient-Centered Quality and Safety • Uses knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional to provide care to patients • Performs history and physical assessments of patients that are accurate, complete, concise, relevant, and appropriate for age, gender, and clinical symptoms. • Provides patient counseling; inclusive of pregnancy prevention, STI Prevention/safer sex practices, contraceptive care counseling and medication management • Counsels and educates patients and families on health maintenance and respond to patient care inquiries • Evaluates and treats patients' medical needs with safe and high-quality treatment • Documents all patient care within an electronic health record in accordance with current policies and procedures • Works as a member of the clinical care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is always provided for all patients • Provides patient-centered care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness and treatment of disease • Refers to appropriate specialists and patient care components as appropriate • Maintain patient confidentiality in accordance with PHI and HIPAA standards Communication and Interdisciplinary Collaboration • Collaborates with clinical care team to ensure the delivery of high quality, patient centric, longitudinal care •

Experience: Candidate questionnaire must be included on resume. • Active Advance Practice Registered Nurse or Nurse Practitioner license in good standing • Current National Board Certification, Family Practice • A minimum of 1 year of recent Primary Care experience as a Nurse Practitioner/Physician Assistant in either a family medicine or primary care practice preferred.

Education : Completion of master’s degree level Family Nurse Practitioner with current National Board Certification and State of Employment license to practice in the Advanced Practice role required.
