Per Diem Nurse Practitioner (NP) in Middleport, OH

Deliver essential home-based healthcare by conducting comprehensive risk assessments as a Nurse Practitioner serving Medicare Advantage plan members throughout Meigs County. This per diem opportunity allows the flexibility to choose your own schedule and make a direct impact on patient well-being across various communities in the county.

• Minimum weekly or bi-weekly hourly commitment required

Responsibilities:

• Perform thorough annual health assessments for Medicare Advantage members in Meigs County, including Salisbury Township, Pomeroy, Rutland Township, and Chester Township
• Evaluate prior and current medical histories, including any chronic or acute conditions, and conduct medication reviews
• Educate members on their diagnoses, treatment options, and medication regimens
• Identify gaps in care and offer practical recommendations to resolve issues
• Visits typically last about 45 minutes and may also include post-discharge follow-ups as needed
• No prescribing or treatment plan development necessaryfocus on assessment, education, and care coordination

Qualifications:

• Active Nurse Practitioner license in the state of Ohio
• Current CPR certification (BLS)
• National board certification through AANP or ANCC
• MSN degree from an accredited nursing school
• At least 6 months of recent NP experience preferred

You will receive compensation of $80$90 per evaluation plus a mileage stipend and reimbursement for administrative time.
