Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage members.
• Evaluate patients prior and current medical history, including conditions and medication use.
• Deliver clear education to patients regarding their medical conditions and prescribed medications.
• Identify any potential gaps in care and provide actionable recommendations.
• Assessments may include annual health evaluations or post-discharge visits, typically lasting about 45 minutes.
• No development of treatment plans or medication prescribing is required.

Qualifications:

• Active Nurse Practitioner license in Ohio.
• Current CPR (BLS) certification.
• Board Certification from AANP or ANCC.
• MSN degree from an accredited nursing school.
• At least 6 months recent NP experience preferred.

This position offers competitive compensation of $80$90 per completed evaluation, plus additional stipends for mileage and administrative time.
