Take the lead in transforming patient care as a Nurse Practitioner in a rewarding home health setting. This per diem opportunity provides the flexibility to create your own schedule while conducting vital home health risk assessments for Medicare Advantage members across Vance County, including Henderson, Kittrell, and Middleburg.

Schedule & Hours:

• Flexible shifts available Monday through Sunday
• Set your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct annual and post-discharge home health risk assessments in patient residences
• Evaluate patients prior and current medical history, medical conditions, and medications
• Educate patients on medical conditions, medications, and health management strategies
• Identify care gaps and offer recommendations to improve patient outcomes
• No requirement to develop treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Board Certification via AANP or ANCC
• Masters degree in Nursing (MSN) from an accredited program
• Current CPR certification (BLS)
• At least 6 months of recent Nurse Practitioner experience preferred
