Key Responsibilities:

• Complete annual home health risk assessments in members homes
• Review and document medical histories, existing conditions, and medication use
• Educate patients about their health conditions and medication regimens
• Identify gaps in care and provide actionable recommendations to resolve them
• No responsibility for treatment planning or medication prescribing

Qualifications:

• Active and unrestricted Nurse Practitioner license in North Carolina
• Current CPR (BLS) certification
• Board Certification via AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited program
• At least 6 months of experience as a Nurse Practitioner preferred
