Deliver essential healthcare directly to patients homes by conducting risk assessments focused on Medicare Advantage plan members in Carteret County, NC. This per diem opportunity is ideal for Nurse Practitioners with a passion for preventative care, autonomy, and flexible scheduling.

Schedule:

• Flexible shifts available Monday through Sunday
• Ability to set your own schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Complete thorough annual home health risk assessments within patients' residences
• Review and evaluate both prior and current medical histories, ongoing medical conditions, and comprehensive medication lists
• Provide focused education to patients about their medical conditions and medications
• Identify potential gaps in care, recommend solutions, and facilitate coordination of care
• No responsibility for prescribing medications or developing treatment plans

Qualifications:

• Current and unrestricted Nurse Practitioner License in North Carolina
• Active CPR certification (BLS required)
• Board Certification as a Nurse Practitioner through AANP or ANCC
• At least 6 months of Nurse Practitioner experience preferred
• MSN degree from an accredited institution
• Exceptional interpersonal and communication skills
