Core Responsibilities:

• Conduct thorough annual and post-discharge health assessments in patients homes
• Evaluate prior and current medical history, ongoing medical conditions, and medication use
• Offer clear education and guidance about medical conditions and prescribed medications
• Recognize and address care gaps, providing actionable recommendations
• Documentation and assessment focusno developing treatment plans or prescribing medications required

Qualifications & Experience:

• Current, unrestricted Nurse Practitioner license in Florida
• Active BLS CPR certification
• Board Certification through AANP or ANCC
• Minimum of 6 months recent experience as a Nurse Practitioner preferred
• Master of Science in Nursing (MSN) from an accredited program
