Per Diem Home Health Nurse Practitioner in Halls, TN

Bring your clinical expertise to a meaningful setting by providing quality care to Medicare Advantage plan members through home health risk assessments. This per diem opportunity offers the flexibility to craft your schedule with shifts available Monday through Sunday. You'll serve communities throughout Dyer County, including Dyersburg, Newbern, Trimble, and surrounding areas, scheduling visits according to your availability and preferences.

Schedule:

• Flexible shifts available seven days a week
• Self-scheduling for optimal work-life balance
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Conduct comprehensive annual and post-discharge risk assessments in patient homes
• Review and evaluate prior and current medical history, conditions, and medication regimens
• Educate members on health conditions and medication adherence
• Identify care gaps and make actionable recommendations to support member health
• No creation of treatment plans or prescribing required

Desired Qualifications:

• Active Nurse Practitioner license in Tennessee
• Current CPR (BLS) certification
• Board certification (AANP or ANCC)
• Master of Science in Nursing from an accredited institution
• At least 6 months experience as a Nurse Practitioner is highly preferred
