Per Diem NP Home Health - Sewanee, TN

Advance your healthcare career as a skilled provider delivering personalized care in the home health setting. This per diem opportunity involves conducting comprehensive home health risk assessments for Medicare Advantage plan members throughout Franklin County, TNincluding Winchester, Decherd, Estill Springs, and surrounding areas. Each assessment typically takes about 45 minutes, with visits ranging from annual evaluations to post-discharge assessments.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own hours and shifts
• Choose from minimum weekly or bi-weekly hourly commitments

Key Duties:

• Perform annual home health risk assessments for Medicare Advantage members
• Review and evaluate prior/current medical history, existing conditions, and medication usage
• Educate patients regarding their medical issues and prescription regimens
• Detect gaps in care, offering recommendations to improve patient outcomes
• No responsibility for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in TN
• Board Certification via AANP or ANCC
• Current BLS certification (CPR)
• MSN from an accredited nursing institution
• At least 6 months of NP experience preferred

This position offers a competitive per-evaluation structure, with $80$90 per completed assessment, plus a mileage stipend and compensation for administrative time.
