The role offers substantial flexibility, allowing you to create your own schedule and select shifts between Monday and Sunday, with a minimum weekly or bi-weekly hourly commitment. Assignments may include visits in local areas such as Jackson, Medon, or Three Way.

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage plan members
• Thoroughly review medical histories, conditions, and current medication lists
• Provide education to members regarding their health conditions and medication regimens
• Identify and document quality or care gaps, recommending resolution strategies
• No requirement to develop treatment plans or prescribe medications

Qualifications:

• Current Nurse Practitioner License in Tennessee
• Board Certification through AANP or ANCC
• CPR certification (BLS)
• Minimum of 6 months recent experience as an NP preferred
• Master of Science in Nursing (MSN) from an accredited institution
