Key Responsibilities:

• Complete comprehensive home health risk assessments for Medicare Advantage plan members within the assigned areas
• Conduct in-depth evaluations of medical history, current health conditions, and medication use
• Deliver patient-centered education on chronic conditions and medications, promoting wellness and adherence
• Identify and document gaps in care; offer actionable recommendations for improved outcomes
• No requirement to prescribe medications or develop treatment plansfocus on assessment and education

Qualifications:

• Active Nurse Practitioner license in Tennessee
• Board Certification from AANP or ANCC
• Current CPR certification (BLS)
• Master of Science in Nursing (MSN) from an accredited program
• A minimum of 6 months of current nurse practitioner experience preferred
