Schedule:

• Flexible shifts offered Monday through Sunday
• Design your own weekly schedule
• Commit to a minimum number of hours, weekly or bi-weekly

Responsibilities:

• Perform comprehensive home health risk assessments, averaging 45 minutes per evaluation
• Assess prior and current medical histories, existing conditions, and medication regimens
• Educate patients on diagnosed conditions and medication use
• Detect gaps in care and suggest appropriate actions for improvement
• Visits may include annual health assessments or post-discharge check-ins
• No prescribing, no treatment plan developmentall focus is on evaluation and education

Qualifications:

• Licensed as a Nurse Practitioner in WV
• Active BLS (CPR) certification required
• Board certified by either the AANP or ANCC
• At least six months of Nurse Practitioner experience preferred
• MSN from an accredited nursing program
