Per Diem Home Health Nurse Practitioner (NP) - Morrison, Tennessee

Schedule:

• Flexible shifts available Monday through Sunday

Key Responsibilities:

• Conduct home health risk assessments, with each visit averaging about 45 minutes
• Evaluate patients prior and current medical histories, including medical conditions and medications
• Educate patients on their health conditions and medications
• Identify gaps in care and offer recommendations to enhance patient health
• Assessment types include annual evaluations and post-discharge visits
• There is no requirement to create treatment plans or prescribe medication

Qualifications:

• Active Nurse Practitioner license in Tennessee
• Board Certification (AANP or ANCC)
• MSN degree from an accredited nursing school
• Current CPR/BLS certification
• At least 6 months of recent Nurse Practitioner experience (preferred)
