Per Diem Home Health Nurse Practitioner in Choctaw, OK

In this role, you will:

• Conduct thorough, 45-minute annual and post-discharge risk assessments for Medicare Advantage members in communities such as Oklahoma City, Edmond, and Midwest City
• Review and evaluate patients previous and current medical histories, existing conditions, and medication regimens
• Deliver clear education regarding diagnoses, treatment options, and medications, ensuring patients understand their care plans
• Identify gaps in care and share recommendations to help close those gaps and improve overall health outcomes
• Focus solely on assessmentno development of treatment plans or medication prescribing required

Qualified candidates will have:

• Active nurse practitioner license in Oklahoma
• Current CPR (BLS) certification
• National board certification through AANP or ANCC
• A Master of Science in Nursing from an accredited institution
• At least 6 months experience as a nurse practitioner preferred
