Key Responsibilities:

• Perform comprehensive annual risk assessments in patients homes within the area, including Chandler, Stroud, Prague, and surrounding communities
• Review and assess previous and current medical history, medical conditions, and medications
• Educate patients about their health status and prescribed treatment regimens
• Identify gaps in patient care and offer practical recommendations for improvement
• Focus exclusively on assessmentsno treatment plan creation or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in Oklahoma
• Current BLS (CPR) certification
• Board certification by AANP or ANCC
• At least 6 months of recent NP experience (preferred)
• MSN from an accredited School of Nursing
