Per Diem Home Health Nurse Practitioner (NP) - Coweta, Oklahoma

Leveraging your clinical skills, you will visit Medicare Advantage members in their homes and provide valuable health evaluations, education, and recommendations to enhance their well-being.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule
• Minimum weekly or bi-weekly hourly commitments, allowing for excellent work-life balance

Key Responsibilities:

• Conduct annual home health risk assessments for Medicare Advantage plan members
• Travel to member residences within areas like Muskogee, Fort Gibson, and Warner
• Review and assess patients medical histories, existing conditions, and medications
• Deliver patient-centered education regarding health conditions and treatments
• Identify care gaps and suggest actionable recommendations
• No requirement for treatment planning or prescribing medications

Qualifications:

• Valid Nurse Practitioner License in Oklahoma
• Active CPR certification (BLS)
• Board Certification via AANP or ANCC
• At least 6 months current experience as a Nurse Practitioner preferred
• MSN degree from an accredited nursing program
