Step into a pivotal healthcare role, supporting patients in Seminole County through in-home clinical assessments. As a Nurse Practitioner for home health services, you will perform detailed risk evaluations for Medicare Advantage plan members. Each assessment visit lasts approximately 45 minutes and may include annual health reviews or post-discharge evaluations. Youll serve diverse areas throughout the county, including Wewoka, Bowlegs, and Konawa, with flexibility in your daily assignment locations.

Key Responsibilities:
- Conduct comprehensive annual home health risk assessments in patient homes
- Review and document medical histories, current health status, and medications
- Educate patients regarding their diagnoses and prescribed therapies
- Identify care gaps and make actionable recommendations
- No involvement in prescription writing or treatment plan development
Qualifications:
- Active Nurse Practitioner license in Oklahoma
- Board Certification via AANP or ANCC
- Current BLS (CPR) certification
- MSN earned from an accredited nursing program
- Minimum of 6 months of current Nurse Practitioner experience preferred
This role offers a competitive rate of $80-$90 per evaluation, with additional mileage stipend and administrative time compensation included.
