Key Responsibilities:

• Conduct home health risk assessments, lasting approximately 45 minutes, on Medicare Advantage members in various communities within Muskogee County
• Perform thorough evaluations of prior and current medical history, existing conditions, and medication use
• Educate patients on their medical conditions and prescribed medications
• Pinpoint gaps in care and share actionable recommendations to enhance patient outcomes
• Note: This position does not involve developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner License in Oklahoma
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• Preferred: 6+ months of current Nurse Practitioner experience
• Master of Science in Nursing (MSN) from an accredited institution
