Per Diem NP Home Health in Greentown, IN

This per diem opportunity is ideal for a Nurse Practitioner with a passion for community-based care and a flexible approach to scheduling. The focus is on conducting home health risk assessments for Medicare Advantage plan members in Howard County, including Kokomo, Greentown, and Russiaville. Each visit takes approximately 45 minutes, and the nature of your assessment may varyfrom annual wellness checks to post-discharge evaluations.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities

• Perform in-home annual risk assessments for members of a Medicare Advantage health plan
• Review prior and current medical history, ongoing conditions, and medication use
• Educate patients about their health conditions and medications
• Identify care gaps and provide actionable recommendations
• No prescribing or development of treatment plans required

Qualifications

• Current Nurse Practitioner license in Indiana
• CPR certification (BLS required)
• Board certification through AANP or ANCC
• MSN degree from an accredited nursing school
• Minimum of 6 months recent NP experience preferred
