Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling to suit your availability
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform comprehensive home health risk assessments, averaging 45 minutes each, for Medicare Advantage members
• Evaluate patients medical history, current medical conditions, and medication profiles
• Deliver patient education on managing chronic and acute conditions as well as proper medication usage
• Identify any gaps in care and recommend solutions to improve overall health outcomes
• No prescribing of medications or development of treatment plans required

Qualifications:

• Valid Nurse Practitioner license in Arkansas
• Current BLS/CPR certification
• Board certification through AANP or ANCC
• Minimum of 6 months of current experience as a Nurse Practitioner preferred
• Master of Science in Nursing (MSN) degree from an accredited institution
