Per Diem Home Health Nurse Practitioner (NP) - Bald Knob, Arkansas

Nurse Practitioners with a passion for home health are invited to make a direct impact on patient wellness through flexible per diem assignments. Responsible for conducting home health risk assessments, you will visit Medicare Advantage plan members residing in communities throughout Woodruff County, including Augusta, McCrory, and Cotton Plant.

Schedule:

Key Responsibilities:

• Complete annual and post-discharge risk assessments in patients homes
• Evaluate prior and current medical history, review medical conditions, and assess medication history
• Provide patient education regarding medical conditions and prescription usage
• Identify gaps in care and offer actionable recommendations
• No requirement for creating detailed treatment plans or medication prescribing

Qualifications:

• Active Nurse Practitioner license in Arkansas
• Board Certification through AANP or ANCC
• Current CPR (BLS) certification
• MSN from an accredited school of nursing
• At least six months of Nurse Practitioner experience preferred
