As a Nurse Practitioner conducting home visits for Medicare Advantage members in locations such as Arkadelphia, Gurdon, and Amity, you will play a crucial role in assessing health conditions, delivering patient education, and recommending necessary follow-up referrals or care measures.

Schedule:

• Flexible shifts available Monday through Sunday

• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct thorough annual home health risk assessments for Medicare Advantage plan members
• Review and evaluate medical history, current healthcare conditions, and medication regimens
• Deliver patient education regarding chronic conditions, disease management, and prescribed medications
• Identify gaps in care and provide referrals or recommendations to address health needs
• No requirement for developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license in Arkansas
• Board certification through AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited institution
• Current CPR/BLS certification
• Minimum of 6 months experience as a Nurse Practitioner (preferred)
