Schedule:

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage members
• Review and document patients medical history, existing conditions, and medication regimens
• Deliver clear education to members regarding their conditions and prescribed medications
• Identify gaps in care and recommend appropriate next steps
• No responsibility for developing treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner license (AR)
• Board certification (AANP or ANCC)
• MSN from an accredited program
• Valid BLS certification (CPR)
• At least 6 months of NP experience (preferred)
