Per Diem NP Home Health New Castle, Colorado

A skilled Nurse Practitioner is needed for per diem work conducting home health risk assessments across Garfield County, Colorado. This is an excellent opportunity to deliver high-quality, patient-focused care directly within the home environment, with significant flexibility in your schedule.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own work schedule
• Minimum required commitment on a weekly or bi-weekly basis

Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage plan members residing in Garfield County, including Glenwood Springs, Rifle, and Carbondale
• Conduct thorough evaluations of past and current medical history, medical conditions, and medication usage
• Educate patients on their medical conditions and prescribed medications to promote understanding and improved compliance
• Identify care gaps in patient histories, and offer appropriate recommendations to address them
• Visits may include annual health assessments or post-discharge evaluations as needed
• No requirement to prescribe medications or create treatment plans

Qualifications:

• Active, unencumbered Nurse Practitioner license in Colorado
• Current CPR (BLS) certification
• Board Certification via AANP or ANCC
• Minimum of 6 months of current NP experience is preferred
• Master of Science in Nursing (MSN) from an accredited institution

This position offers compensation of $80-$90 per completed evaluation, plus a mileage stipend and payment for administrative time.
