You will perform risk assessments and health evaluations for Medicare Advantage plan members throughout Garfield County, including Glenwood Springs, Rifle, Carbondale, and surrounding areas.

• Flexible shifts available Monday through Sunday

• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
• Review and evaluate patients prior and current medical histories, chronic conditions, and medications
• Deliver thorough patient education regarding health conditions and medication management
• Identify gaps in care and provide thoughtful recommendations for follow-up
• Focus solely on assessmentsno prescribing of medication or creation of treatment plans required

Qualifications:

• Active Nurse Practitioner license in Colorado
• Valid CPR certification (BLS)
• Board certification through AANP or ANCC
• Minimum of six months current experience as a Nurse Practitioner preferred
• MSN degree from an accredited nursing program
