Experience the rewards of home health by conducting in-person risk assessments for Medicare Advantage members across Mesa County, Colorado. As a Nurse Practitioner, youll have the autonomy to manage your schedule while making a tangible impact on the lives of patients in their own homes.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own work schedule
• Minimum hourly commitment required on a weekly or bi-weekly basis

Responsibilities:

• Perform thorough annual risk assessments for Medicare Advantage plan members, including reviews of prior/current medical and medication history
• Educate patients on management of chronic and acute medical conditions
• Identify care gaps and offer targeted recommendationsno prescription writing or treatment plan development needed
• Each visit typically spans 45 minutes and may be for an annual assessment or post-discharge follow-up
• Travel within Grand Junction, Fruita, Palisade, and greater Mesa County as scheduled

Qualifications:

• Active Nurse Practitioner license in Colorado
• Masters degree in Nursing (MSN) from an accredited institution
• National board certification (AANP or ANCC)
• Current BLS (CPR) certification
• At least six months recent NP experience (preferred)
• Strong communication and assessment skills; ability to engage compassionately with a diverse patient population
