Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum hourly commitment required on a weekly or bi-weekly basis
Responsibilities:
- Perform thorough annual risk assessments for Medicare Advantage plan members, including reviews of prior/current medical and medication history
- Educate patients on management of chronic and acute medical conditions
- Identify care gaps and offer targeted recommendationsno prescription writing or treatment plan development needed
- Each visit typically spans 45 minutes and may be for an annual assessment or post-discharge follow-up
- Travel within Grand Junction, Fruita, Palisade, and greater Mesa County as scheduled
Qualifications:
- Active Nurse Practitioner license in Colorado
- Masters degree in Nursing (MSN) from an accredited institution
- National board certification (AANP or ANCC)
- Current BLS (CPR) certification
- At least six months recent NP experience (preferred)
- Strong communication and assessment skills; ability to engage compassionately with a diverse patient population
