Per Diem Home Health Nurse Practitioner in Littleton, CO

Seeking a skilled Nurse Practitioner to perform home health risk assessments within Jefferson County, CO, including the Lakewood, Arvada, and Westminster areas. This per diem position offers outstanding flexibility and autonomy, allowing you to create your own schedule, with shifts available any day between Monday and Sunday. Schedule:

• Flexible shifts throughout the week, Monday through Sunday
• Set your own schedule
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Perform home-based, annual health risk assessments for Medicare Advantage plan members
• Evaluate and document prior and current medical history, medical conditions, and medication usage
• Educate patients on their medical conditions and prescribed medications
• Identify care gaps and provide recommendations to help improve overall health outcomes
• No development of treatment plans or prescription of medications required
• Average assessment duration is approximately 45 minutes per visit

Qualifications:

• Active Nurse Practitioner License in Colorado
• Current BLS CPR certification
• Board-certified through AANP or ANCC
• At least six months of recent Nurse Practitioner experience is preferred
• Master of Science in Nursing (MSN) from an accredited institution
