Per Diem Nurse Practitioner Home Health Clifton, Colorado

Experienced Nurse Practitioners are sought to conduct home health risk assessments within Mesa County, CO. This per diem opportunity focuses on flexible, independent work performing comprehensive evaluations for Medicare Advantage plan members residing in areas such as Grand Junction, Fruita, and Palisade. Each assessment typically takes about 45 minutes and may vary from annual wellness exams to post-discharge visits.

Schedule:

• Flexible shifts available Monday through Sunday
• Freedom to create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform annual home health risk assessments, gathering detailed medical, surgical, and medication histories
• Evaluate current and prior medical conditions for each member
• Educate members about their medical conditions and medication regimens
• Identify gaps in care and provide actionable recommendations
• Document findings without the need for treatment planning or prescribing medications

Qualifications:

• Active Nurse Practitioner license in the state of Colorado
• Current BLS certification
• Board Certification through AANP or ANCC
• Minimum of 6 months' Nurse Practitioner experience preferred
• MSN degree from an accredited nursing program
