Per Diem Home Health Nurse Practitioner (NP) - Georgetown, DE

Make a meaningful impact in the community by providing comprehensive home health risk assessments as a Nurse Practitioner. In this per diem position, you will visit Medicare Advantage plan members across Sussex County, DEincluding Milford, Seaford, and Georgetowndelivering high-quality care in a home setting.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own work schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members
• Assess medical history, current medical conditions, and medication usage
• Offer education on health conditions and medication management to patients and caregivers
• Identify gaps in care and provide practical recommendations to improve patient outcomes
• Visits are approximately 45 minutes each and focus on holistic assessment (no treatment planning or prescribing required)

Qualifications:

• Current Nurse Practitioner license in Delaware
• Board Certification through AANP or ANCC required
• Valid BLS/CPR certification
• MSN degree from an accredited nursing program
• Minimum of 6 months experience as a Nurse Practitioner preferred

Compensation for this role includes $80$90 per evaluation, along with a mileage stipend and payment for administrative time, providing excellent earning potential for self-motivated professionals.
