Advance Practice Provider (NP /PA), Primary Care - Mon-Fri, no weekends/eves, 6-8 patients per day Full-time, Monday - Friday, no weekends or evenings. Manage chronic conditions of our own patient panel seeing 6-8 patients per day on average. For over 30 years, HarmonyCares has been delivering home-based care. Responsibilities : The APPs we seek are those who have directly related experience in home health, geriatrics, hospice and primary care. The APP who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient's needs.
