This provider will take over and maintain a panel of patients for all acute, chronic and health maintenance needs.

- Practice alongside 10 primary care physicians and one fellow primary care APP
- Primarily Family Medicine Practice seeing all ages
- Strong access to a robust specialty referral network in the building and region
Duties & Responsibilities
- 36 patient-facing hours and four admin hours/week
- Monday-Friday with 8-, 10- or 12-hour shift flexibility
- Hours to include a combination of two early or two late clinics (or combo) per week
- Up to two Saturday morning clinics/month (provider choice)
- 4 weeks of phone-based call per year
- 100% outpatient
Preferred Qualifications:
- Applicants with previous Advanced Practice Provider experience in primary care setting is preferred, not required. Relevant RN experience may be considered.
Position Requirements:
Nurse Practitioner:
- Current licensure from the State of Illinois as a Registered Professional Nurse and Advanced Practice Nurse or eligible to obtain a licensure through the Illinois Department of Financial and Professional Regulation (IDFPR) application process
- Master of Science in Nursing or Doctorate of Nursing practice with clinical specialty appropriate to position
- Current certification by American Nurse Credentialing Center (ANCC) or American Academy of Nurse Practitioner (AANP)
- CPR, ACLS, and PALS certifications
Physician Assistant:
- Current licensure from the State of Illinois as a Physician Assistant/Associate or eligible to obtain a licensure through the Illinois Department of Financial and Professional Regulation (IDFPR) application process
- Successful completion of an accredited Physician Assistant/Associate Program
- Current certification from National Commission on Certification of Physician Assistants
- CPR, ACLS, and PALS certifications

Rush North & Harlem is significantly expanding access to Rush primary and specialty care for northwest Chicago and suburban patients in near west communities like Oak Park, River Forest and Elmwood Park, by offering some of the most sought-after healthcare services - including nationally ranked specialties like neurology and cardiology. The new center is another example of how Rush is working to deliver the best care and outcomes across the Chicago area and Northwest Indiana.
The Advanced Practice Provider (APP) will promote collaboration between multiple disciplines to ensure cost effective and quality patient care. The individual who holds this position will exemplify the Rush mission, vision and values and act in accordance with the policies, procedures, guidelines of Rush, including the Rush Medical Group Customer Service standards. Successful candidates will also meet all other state and federal regulatory practice and accreditation standards.
Rush has over 600 APPs across the enterprise that provide comprehensive patient care each day autonomously and in collaboration with other members of the health care team. Across the system, the APPs are supported by 4 levels of leadership, including 39 Lead APPs, three chief CRNAs, and a Senior Director of Advanced Practice Providers. Rush offers competitive total compensation that includes base pay, benefits, and other incentive programs for our employees. The total pay range shown reflects the wide range of factors that are considered in making compensation decisions including, but not limited to, specialty; relevant experience and training; and other business and organizational factors. This total pay range is for a full-time, 1.0 FTE employee, and includes any incentive payments that may be applicable to this role. Additional compensation may include premium pay rates, sign-on bonuses, and relocation allowances. We also offer a comprehensive benefits package.
Pay Range: $132,000 - $200,000
