Geriatrics Physician Assistant

We are seeking an experienced NursePractitioner or Physician Assistant for a Locums assignment to convert toPerm (within 3-6 months) at a primary care practice serving geriatric patients ina value-based care model. This is a hybrid role with blend ofcoverage in outpatient clinics as well as home-based patient care.

Compensation: $75 - $95 perhour

Position Details:

• Schedule: Full Time Hours Guaranteed
• Monday - Friday - 5 Days Per Week
• Patient Load: LOW DAILY CENSUS
• Home Care: 3-5 patients per day
• Clinic: Small census of 7-10 patients per day
• Setting: Hybrid (Outpatient clinic + home-based care), value-based primary care for elderly patients

Responsibilities:

• Deliver comprehensive primary care services, including preventive care, chronic disease management, and acute care to Geriatric patients
• Diagnose, treat, prescribe medications, and develop individualized treatment plans
• Order and interpret diagnostic tests (labs, imaging, etc.)
• Document accurately in the EHR system
• Provide patient education and promote wellness initiatives
• Collaborate with physicians, nurses, and support staff to ensure coordinated, high-quality care
• Refer patients to specialists and manage follow-ups
• Ensure compliance with state, federal, and professional regulations

Requirements:

• Active NP or PA license in NY
• Minimum 2 years of primary care experience
• National Board Certification
• Spanish Speaking Preferred
• Expedited Credentialing
• Medicaid and Medicare Provider Id required
