Per Diem Nurse Practitioner (NP) - Versailles, OH

Key Responsibilities:

• Conduct annual home health risk assessments for Medicare Advantage plan members
• Review prior and current medical history, medical conditions, and medication lists
• Educate patients about their medical conditions and medications
• Identify any gaps in care and provide expert recommendations for improvement
• Complete comprehensive documentation for each assessment
• No prescribing or treatment plan creation required

Qualifications:

• Current Nurse Practitioner License in Ohio
• Active CPR certification (BLS required)
• Board Certification through AANP or ANCC
• Minimum of 6 months of current NP experience preferred
• MSN obtained from an accredited school of nursing
