We are seeking a dediced Hospice Nurse Practitioner to perform Face-to-Face (F2F) encounters essential for hospice eligibility certificion and recertificion. In this role, you will gher comprehensive clinical informion, perform detailed assessments, and collabore closely with hospice physicians and interdisciplinary teams to ensure quality pient care and regulory compliance.
Key Responsibilities:
Perform and complete physical assessments for hospice F2F encounters within CMS-established regulory timeframes.
Document clinical findings accurely and test F2F encounters in pient medical records.
Utilize Local Coverage Determinion (LCD) criteria per hospice policy and regulory requirements.
Communice clinical findings, review trement plans, lab results, diagnostics, and medicions with certifying physicians.
Collabore effectively with physicians, interdisciplinary group (IDG) members, pients, caregivers, and families.
Provide clinical recommendions to the IDG based on F2F assessment findings.
Conduct assessments in the pients place of residence, including home, skilled nursing, or other settings.
Notify the hospice agency promptly if unable to complete F2F visits as scheduled.
May provide F2F coverage for multiple provider numbers with approval.
Compension may vary within the salary range provided based on several factors including but not limited to a candides locion, experience, educion, skills, licensure, certificions and department equity. Gentiva provides associes with a comprehensive benefits and total rewards package, of which base pay is just one piece.

Qualificions Wh Youll Bring:
Masters or higher degree in Nursing
Completion of an accredited gradue-level nurse practitioner educion program
Current registered nurse license in ste of practice
Current advanced practice nurse license or certificion
Nional Nurse Practitioner certificion (ANCC or AANP)
Collaborive agreement in place as required
Current CPR certificion
Valid drivers license and automobile insurance
Strong understanding of hospice care principles and terminally ill pient support systems
Excellent pient assessment and clinical evaluion skills
Familiarity with LCD criteria and hospice documention requirements
Knowledge of Medicare, Medicaid, ACHC, and other regulory guidelines
Strong oral and written communicion skills
Compassione and sensitive to end-of-life issues impacting pients and families
Ability to provide emotional support during times of crisis
Flexible and adaptable with ability to manage diverse tasks efficiently
Effective communicor across diverse socioeconomic and cultural backgrounds
Cooperive and tactful team member adhering to OSHA, ste, and federal hospice regulions
Preferred (Not Required):
Experience in acute care, hospital, oncology, home health, skilled nursing, or hospice settings
