Per Diem Home Health Nurse Practitioner (NP) - Smyrna, GA

This per diem opportunity is ideal for a Nurse Practitioner seeking autonomy and a flexible schedule.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-schedulingset your own work hours
• Minimum weekly or bi-weekly hourly commitment to maintain active status

Key Responsibilities:

• Complete in-home risk assessments for Medicare Advantage plan members
• Gather and evaluate prior and current medical histories, chronic conditions, and medication usage
• Educate members about their health conditions and current medications
• Identify gaps in care and deliver actionable recommendations
• No requirement to develop treatment plans or prescribe medication

Qualifications:

• Active Nurse Practitioner License in GA
• Current CPR (BLS) certification
• Board Certification through AANP or ANCC required
• MSN from an accredited nursing program
• At least 6 months of current NP experience preferred

Compensation for this role is $80-$90 per completed evaluation, in addition to a mileage stipend and payment for documented administrative time.
