This per diem role involves traveling to various locations within Baldwin County, including Milledgeville, Ivey, and Deepstep, to conduct home health risk assessments for Medicare Advantage plan members. Key Responsibilities:

• Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
• Review and evaluate patients medical and medication histories
• Educate patients regarding chronic conditions, preventive measures, and prescribed medications
• Identify gaps in care and provide clear recommendations to support better health outcomes
• No requirement to develop treatment plans or prescribe medications

Qualifications:

• Current Nurse Practitioner license in the state of Georgia (GA)
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN degree from an accredited school of nursing
• At least 6 months of recent NP experience (preferred)
