Schedule: Monday-Friday 8:00am- 5:00pm with occasional weekday- afterhours and weekend on-call.

Position Summary:

The Nurse Practitioner- Hospice Care provides advanced nursing care and serves as a clinical expert in end-of-life care. This role delivers direct patient care and evaluates outcomes across settings, including patients’ homes, hospice care centers, and nursing facilities. Working collaboratively with multidisciplinary healthcare teams, the Nurse Practitioner addresses the medical, psychosocial, and spiritual needs of patients, families, and caregivers. The Nurse Practitioner develops, updates, and evaluates individualized treatment plans, including medications and other care interventions, in accordance with physician orders, regulatory requirements, and organizational policies.

Th is position will work closely with Bluegrass Care Navigators’ physicians and community providers, following established palliative and symptom management protocols, while maintaining professional expertise and serving as a role model in advanced nursing practice. This position ensures high-quality, compassionate care that aligns with the hospice philosophy and supports patients and families throughout the end-of-life journey.

Position Qualifications:

  • Graduate of a master’s degree program in nursing.
  • APRN certification as a Family or Adult Nurse Practitioner.
  • Current APRN licensure in good standing with the State Board of Nursing.
  • Minimum of three (3) years of general nursing experience; experience in hospice, home health, medical-surgical, or critical care settings preferred.
  • Skilled in nursing practice, including pain management; IV skills preferred.
  • Knowledgeable in area of specialty with ability to translate professional and organizational goals into effective practice.
  • Committed to evidence-based practice and nursing research, with ability to apply findings to clinical care.
  • Strong organizational skills, able to manage multiple priorities and resources effectively.
  • Excellent interpersonal, communication, collaboration, and conflict resolution skills.
  • Knowledge of healthcare systems and change management processes.
  • Ability to handle emotional stress and complex patient/family situations with professionalism and compassion.
  • Complies with accepted professional nursing standards and principles.
  • Understands and supports Bluegrass Care Navigators’ philosophy of hospice care and principles of end-of-life care.
  • Provides timely , accurate , and detailed documentation; demonstrates strong nursing and medical judgment.
  • Possesses and maintains current CPR certification.
  • Self-directed, flexible, and able to work with minimal supervision within an interdisciplinary team.
  • Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements.
