Per Diem Home Health Nurse Practitioner in Shelley, ID

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling: design your own workweek
• Minimum weekly or bi-weekly hourly commitment required

Responsibilities:

• Conduct in-home risk assessments for Medicare Advantage health plan members
• Review and evaluate medical history, current and prior health conditions, and medication usage
• Educate members about their medical conditions and prescribed treatments
• Identify gaps in care and recommend steps for improvement
• Complete thorough assessments without prescribing medications or creating treatment plans

Qualifications:

• Current Nurse Practitioner license in Idaho
• Active CPR certification (BLS)
• Board certified through AANP or ANCC
• Minimum of 6 months recent NP experience preferred
• MSN from an accredited nursing program

This role provides compensation of $80$90 per evaluation, plus a mileage stipend and paid time for administrative work.
