Schedule:

Key Responsibilities:

• Perform annual home health risk assessments in patients homes, generally completed in about 45 minutes each
• Review and evaluate medical history, current conditions, and medication use
• Educate patients regarding diagnoses and medication management
• Identify care gaps and recommend solutions to improve health outcomes
• No responsibilities for treatment planning or medication prescribing

Qualifications:

• Current Nurse Practitioner license in Idaho
• Completion of an MSN from an accredited nursing program
• Active board certification (AANP or ANCC)
• Current BLS (CPR) certification
• At least 6 months of recent Nurse Practitioner experience preferred
