Per Diem Home Health Nurse Practitioner (NP) - Buhl, Idaho

Deliver impactful patient care as a Nurse Practitioner conducting home health risk assessments for Medicare Advantage members throughout Jerome County. This per diem position offers exceptional flexibility, allowing you to craft your own schedule and select shifts that fit your availability, with opportunities to serve residents in locations such as Jerome, Hazelton, Eden, and surrounding areas.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule
• Weekly or bi-weekly minimum hourly commitment

Key Responsibilities:

• Perform comprehensive annual home health risk assessments (approximately 45 minutes each) in members homes
• Review and assess prior/current medical history, existing medical conditions, and all medication histories
• Offer targeted education to patients regarding their health conditions and prescribed medications
• Identify gaps in care and suggest appropriate solutions to address those needs
• Focus solely on patient assessmentno prescribing or development of treatment plans required

Qualifications:

• Active Nurse Practitioner license in Idaho
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• Masters degree in Nursing (MSN) from an accredited institution
• Minimum of six months of current Nurse Practitioner experience preferred

Practitioners in this role receive compensation at $80$90 per completed evaluation, along with a mileage stipend and additional pay for administrative duties.
