Key Responsibilities:

• Conduct annual and post-discharge home health risk assessments for Medicare Advantage members
• Review and evaluate patients medical histories, conditions, and medication lists

• Role does not require prescribing medications or developing treatment plans

Qualifications:

• Active Nurse Practitioner license in Idaho
• Board Certification via AANP or ANCC
