Key Responsibilities:

• Conduct annual home health risk assessments, each lasting approximately 45 minutes
• Travel to various locations within Columbiana County, including Perry Township, Salem, and East Liverpool
• Review and document comprehensive medical history, medical conditions, and medication use
• Educate patients about their medical conditions and medications, helping them make informed health decisions
• Identify gaps in care and offer actionable recommendations for ongoing patient wellness
• No requirement to develop treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Ohio
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• At least 6 months of NP experience is preferred
• MSN from an accredited nursing program
