Your clinical expertise will help identify care gaps and enhance the well-being of individuals in their homes.

Key Responsibilities:

• Conduct thorough home health risk assessments for Medicare Advantage members
• Review medical, medication, and treatment histories
• Provide patient education regarding conditions and medication regimens
• Identify and address potential care gaps, offering recommendations as needed
• Documentation and reporting for each assessment
• No requirement to prescribe medications or develop treatment plans

Qualifications:

• Valid Nurse Practitioner License in Ohio
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• MSN degree from an accredited nursing program
• Six months of recent NP experience preferred
