Per Diem Nurse Practitioner Home Health Lisbon, Ohio

Deliver essential home-based care by conducting risk assessments for Medicare Advantage plan members across Columbiana County, including towns such as Salem, Perry Township, and East Liverpool. This per diem opportunity allows you to leverage your expertise as a Nurse Practitioner in a flexible, autonomous role while making a meaningful difference in each patients well-being. Each in-home assessment will typically last around 45 minutes and may encompass either annual health assessments or post-discharge evaluations.

Schedule:

• Flexible shifts offered Monday through Sunday
• Create your own schedule
• Weekly or bi-weekly hourly commitments available

Core Responsibilities:

• Perform comprehensive annual home health risk assessments for Medicare Advantage health plan members
• Review and document patients prior/current medical history, conditions, and medication use
• Offer patient education regarding disease processes and medication management
• Identify and address gaps in care, providing recommendations as appropriate
• No need to develop treatment plans or prescribe medications

Preferred Qualifications:

• Active Nurse Practitioner license in Ohio
• Current CPR (BLS) certification
• Board Certification via AANP or ANCC
• 6 months of recent NP experience is ideal
• MSN from an accredited nursing program
