This per diem role offers experienced Nurse Practitioners the opportunity to deliver essential care through home health risk assessments for Medicare Advantage members. Working within Shelby County, including communities such as Sidney, Fort Loramie, and Anna, you'll be responsible for conducting assessments in patients homestypically lasting around 45 minutesfocused on annual health evaluations or post-discharge check-ins.
Schedule:
- Flexible shifts available seven days a week
- Create and manage your own schedule
- Commitment required on a weekly or bi-weekly basis
Key Responsibilities:
- Complete comprehensive home health risk assessments for Medicare Advantage members
- Review and document prior/current medical history, conditions, and medication use
- Educate patients on their medical conditions and prescribed medications
- Identify potential care gaps and offer recommendations for resolution
- No responsibility for creating treatment plans or prescribing medications
Desired Qualifications:
- Active Nurse Practitioner license in Ohio
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Current BLS CPR certification
- At least 6 months of Nurse Practitioner experience preferred
This position offers $80-$90 per completed evaluation, in addition to a mileage stipend and administrative time compensation.
