As a key healthcare provider, you will play a crucial role in supporting Medicare Advantage plan members in their homes. Schedule:

• Flexible shifts available Monday through Sunday
• Self-directed scheduling; create your own work calendar
• Minimum hourly commitment required weekly or bi-weekly

Key Responsibilities:

• Conduct in-home annual and post-discharge risk assessments for Medicare Advantage members
• Review and evaluate medical history, ongoing conditions, and current medications
• Provide patients with clear education about their medical conditions and medications
• Identify gaps in care, offering recommendations to improve overall health outcomes
• Documentation required only; no treatment planning or prescriptions needed

Qualifications:

• Active Nurse Practitioner license in the state of Georgia
• Current CPR certification (BLS required)
• Board Certification as NP (AANP or ANCC)
• Completion of a Master of Science in Nursing from an accredited institution
• At least six months of recent Nurse Practitioner experience preferred

This role offers compensation of $80$90 per patient evaluation, which includes a mileage stipend and reimbursement for administrative time.
