Per Diem Home Health Nurse Practitioner (NP) - Roberta, GA

Experienced nurse practitioners passionate about home health and preventive care are invited to deliver quality care through one-on-one visits with Medicare Advantage members. This per diem opportunity focuses on conducting home health risk assessments in Taylor County, GA, including Butler, Reynolds, and Howard. As a valued caregiver, you will play a pivotal role in supporting patients health by delivering thorough assessments and meaningful education during home visits.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Design your own work schedule
• Minimum commitment required (weekly or bi-weekly)

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage members
• Review medical history, current conditions, and medications
• Educate patients on medical conditions and medication usage
• Detect any gaps in care, making recommendations as necessary
• Visits may include annual health assessments or post-discharge follow-up
• No requirement to devise treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Georgia
• Current Basic Life Support (CPR) certification
• Board Certification by AANP or ANCC
• At least six months of recent experience as an NP is preferred
• Master of Science in Nursing (MSN) from an accredited institution
