Per Diem Home Health Nurse Practitioner (NP) - Talbotton, Georgia

An opportunity is available for a Nurse Practitioner skilled in Home Health to perform risk assessments on a per diem basis. In this role, you will visit Medicare Advantage plan members residences within Taylor County, GAcovering locations such as Butler, Reynolds, and Howard. Each assessment typically lasts about 45 minutes and may involve annual health check-ups or post-discharge evaluations.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct annual and post-discharge home health risk assessments for Medicare Advantage members
• Review and evaluate each patients previous and current medical history, conditions, and medication lists
• Educate members about their medical conditions and proper medication management
• Identify potential gaps in care and provide actionable recommendations to address them
• Note: No requirement to prescribe medications or develop treatment plans

Qualifications:

• Active Nurse Practitioner License in Georgia
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN from an accredited nursing program
• At least 6 months of recent Nurse Practitioner experience preferred

This per diem position offers $80-$90 per completed evaluation, plus a mileage stipend and compensation for administrative time.
