Key Responsibilities:

• Conduct home-based annual health risk assessments within Baldwin County communities, including Milledgeville, Ivey, and Deepstep
• Evaluate patients current and past medical history, conditions, and medication usage
• Educate members about their health conditions, medications, and proactive care practices
• Identify gaps in care and recommend appropriate solutions or resources
• No requirement to develop treatment plans or prescribe medications

Qualifications & Experience:

• Active Nurse Practitioner license in Georgia
• Board certification (AANP or ANCC)
• Current CPR (BLS) certification
• MSN degree from an accredited nursing program
• At least 6 months of Nurse Practitioner experience preferred
