As a Nurse Practitioner in this role, youll engage with patients in their homes across Douglasville, Villa Rica, and Austell, providing critical assessments that help shape patient wellness and care coordination.

Key Responsibilities:

• Perform comprehensive annual health risk assessments for Medicare Advantage members in the community
• Review and evaluate patients prior and current medical histories, conditions, and medication usage
• Deliver patient education regarding medical conditions and medication regimens
• Identify gaps in care and offer recommendations to ensure comprehensive coverage and improved outcomes
• No direct prescribing of medications or development of treatment plans required

Qualifications:

• Active Nurse Practitioner License in Georgia
• Board Certification through AANP or ANCC
• Current BLS (CPR) certification
• Minimum of 6 months current Nurse Practitioner experience (preferred)
• Master of Science in Nursing (MSN) from an accredited institution
